Live Event Request
For single, Live Event requests. All these will be checked for approval and confirmation.

COTM Events Request
Use this form to send in your COTM list.

*If desired you can email the list to Partyline, according to the form's format (see COTM guidelines for more on this).
*This form is to be used only to send in a large number of events you already put together for your COTM.
*Any additions, changes, removals or events added after the original list has been sent should go through the Live Event request form *not* COTM list
  (unless there is a list of changes/additions). Just please make sure this list is as complete as possible before sending it.
